Tendinitis is annoying. Of course, I don't have the close medical care Kyrie has, but I've been dealing with it for 6 months in my foot. When I was off my feet for a month it didn't bother me at all and doesn't bother me at all until I do the types of thing (various forms of exercise) that aggravate it... then it barks loudly. He was probably feeling good on it because he just couldn't simulate game conditions and once he did, it returned. Sounds pretty typical of it.
Seems like Kyrie needs to learn how to play in a way that he avoids aggravating it. If it's hard stops, then stop doing them. Maybe he just needs to be a spot up 3 guy who can go to more of a runner if there's a hard close out. Of course, it may be that the things aggravating are things he has to do, and if so, then he (and we) are outta luck.
