I can only guess that maybe the 1st base umpire had a bad angle and thought Lively's legal look over to third base was something else?
 
The only thing I can think of - there may have been an ever-so slight bob of his head and upper body.

Yeah, it’s either when he inhaled deeply or the look over to 3rd. Both completely legal, but otherwise I can’t see anything that even comes remotely close to a balk.

He moved his head. Shoulders didn't move, hips weren't moving, no leg twitch or anything. I don't see it.
 
One of the most egregiously awful calls I've seen in recent years. The first base ump had no business calling a balk there.
