Joint/Foot Pain

I've spent far too much time on WebMD lately and figured RCF probably has a few Dr's, so I figured I'd take a shot.

My first symptoms started back in February with occasional, intermittent chest tightness. Had a chest xray, EKG, wore a heart monitor - all normal.

About two months ago my left knee started bothering me a little. Nothing awful, just bugged me a bit during physical activity and when I would go to get up. Chalked it up to getting closer to 40.

About a month ago though, my legs/feet below the knee started tingling for a few days. One day that turned into more of a burning sensation in my feet, which subsided after about 48 hours. Since that time, I've felt pain in my feet/ankles to varying degrees, my knee still bothers me, and over the past few days I've experienced mild soreness in both wrists. I went to my podiatrist and he said plantar fasciitis in both feet, but it seems like there's more going on.

My doctor suggested Naproxin OTC, which helps but hasn't eliminated the symptoms. He did bloodwork too which found a weak positive ANA but nothing else. I've also lost 30 lbs since January, however, I have been dieting/exercising since that time too.

Dr. Google brought me to Rheumatoid Arthritis, but it also seems that many things can mimic RA. I have appointments coming up with both a neurologist and rheumatologist, but figured I'd see if anyone had input in the meantime.
 
I'm not a professional but I an going with weightloss related gout. Increased purines in the blood leads to build up of uric acid in the joints.

Interesting, I always thought gout was tied to weight gain. Thanks for the input.

The burning sensation in my feet was ongoing for about 24-48 hours then went away for the most part. My persistent symptoms have been foot/ankle soreness/pain (both sides) and left knee soreness (haven't felt it much in the right one). I've also had some calf tightness and mild back pain.

It hasn't been debilitating (I played baseball with my son today) but has definitely impacted me.

That is good that it went away. Had it persisted one might suspect a neurological issue.

Have you tried orthotics? A heavy stretching regimen?

You sound like me in Army. The impact of running 20+ miles every week, and weight carried, took a toll, but good orthotics and stretching helped a bit.

I bought orthotics yesterday...haven't tried heavy stretching to this point. I kind of assumed it was a disease/disorder like RA instead but maybe I'm wrong!
 
Chest tightness could be just a pinched nerve or pulled muscle. It can be super scary because it can feel like it is actually your heart that is tight. Google that and try some stretches, I forget exactly what muscle/nerve you need to target, but in the past I have freaked out about it, then did some stretches to recognize that I'm not having a heart attack, it's just a nerve/muscle since the stretching alleviates it.

The rest does kind of sound like arthritis but I'm no expert there. The symptoms (tingling, joint pain) are similar to my mothers' when she was diagnosed with arthritis.

Can you summarize your diet and give a basic description of your exercise regimen?
 
Not sure if similar situation to what I was in a few months ago but I had similar experiences. Lost between 25-30 pounds, started working out twice a day and increased my Protein intake. I had some pretty significant gout like symptoms and my doctor told me it could be from the protein shakes I was taking (ingredients inside the mix).

I started in January right after the new year. Typically a half salad for lunch, whatever my wife made for dinner (chicken, roast, burgers, steak, etc) but in smaller portions than I used to do, and a small snack at bedtime. Prior to this I was pretty bad eating - I'm not proud to say it, but I could knock out a bag of chips in a day. For exercise I joined a rec softball team that played twice a week and made sure to do at least 30-45 minutes of exercise per day, even if it was just shooting around outside. I went from 210 to 180 from January to now, and have stayed around that weight over the past two weeks or so.

Where did the symptoms impact you most? Mine are almost exclusively feet/ankles. How bad was the pain? I've never experienced gout before but I always thought it was excruciating pain, which I haven't really had (although it's certainly enough pain to impact my day).
 
I've noticed over the last 24-48 hours that my primary discomfort has been in both Achilles and tops of my feet and seems to get worse with inactivity.

Do you have any single joints, generally on your toes, that are very locally red, painful and warm to the touch?
 
Nope, no redness/warm to the touch anywhere.

Probably not gout then.

Gout presents in a specific way, most of the time. I have a couple friends that have it and they each first exhibited with a swollen looking joint, one a finger, the other a big toe.

It can progress to bigger joints but usually starts small.
