I actually really like the 12 with a league expansion. 4 divisions in each league and then the 2 wild cards... I almost always make that setup when I am playing OOTP.

Owners want the 14, but unless the players get a bigger cut of the playoff money, I doubt they will he willing to expand it more.

This gets a bit complicated legally, but the real issue with the antitrust exemption is the minor leagues. A lot of the antitrust stuff disappears in the MLB (and NBA/NFL) situation where you have a union and a multiemployer bargaining group. Repealing the antitrust exemption would likely mean the death of major league teams supporting minor league franchises -- at least anything close to the scope they do today.
