Brief googling led me to acute renal failure associated with inhalation of sea-water/near drowning. I'd imagine if he saved two kids at the same time, he didn't save one and then the other... he grabbed them both, swam while carrying two kids, inhaled a shit-load of water while trying to keep them afloat, had enough strength and oxygen to get to shore, but the oxygen deprivation and ingested water cause him to go into shock and renal failure followed. We have a family friend that unfortunately passed due to a similar situation a year or so ago. Father saved the kids and was able to push them out of the rip tide but was unable to get himself out.
