Thank you. That makes sense- the image I was getting in my head was far more lewd and I wasn't able to reconcile the image in my head with the widespread use of the term.

Although, technically, that picture is not so much depicting the ass as a hat as it is depicting the ass as a helmet. A hat sits on top, a helmet surrounds. That man's asshole is surrounding his head. Sorry if I am getting into semantics.
