Small actions have really huge consequences all the time in this series.
Cat effectively demolished the Riverlands by kidnapping Tyrion in the first book. It's likely that thousands will die come winter as a result of all those burned fields and pillaging.
Robb sending Theon as his envoy to Balon was the reason he ended up losing his war. Without that stupid decision, Winterfell is never taken, his brothers are never presumed dead, he (probably) never ends up marrying Jeyne Westerling and keeps the Freys as his allies, Roose Bolton never has any cause to betray him, etc.
There are plenty of other examples too, but Martin is big on the butterfly effect.
